Custom Form Templates

Portia enables Custom Forms to help you run your business seamlessly. Everything you need to collect for each session is readily available for your team to complete.

There are two categories and six types of Forms in Portia, which have specific purposes.
Learner category forms are specific to the Learner.
Team category forms are specific to the Team Member.
The Learner forms include 5 types of forms:
Learner Profile
Start of Session
In-Session
End of Session
Meeting Forms
and Other

Learner Profile Forms. These forms will appear on the Learner’s Profile Page. Here the Guardian will complete it. These forms are accessible for every Learner and are commonly used to gather more information during Client Intake.

Session Forms are used during one-to-one and group sessions and are completed on the PortiaPro mobile app. These forms include: Start of Session Form, End of Session Form and In-Session Form

Meeting Forms - can only be completed in Meeting Sessions, in PortiaPro, and on the Portia Platform Web Admin

Other - Can be completed on the Portia Platform (web admin only)

Team Forms can be only completed in a Team Member’s Profile. This form type is useful for tracking staff evaluation, supervision hours, and onboarding information.

Forms can include any of the following:
Text, Paragraph, Checkboxes, Multiple Choice, Date, Dropdown List, Time, Number, Email, Address, and Signature
You can select to have an item required. In doing so the Form will not be complete until the required items are filled.
